Chronic care management (CCM). Starting January 1, 2015, CCM payment will cover non-face-to-face care...delivered electronically, by phone, etc...that is documented in the EHR.

Medicare patients are eligible for CCM if they have two or more chronic conditions that put them at risk for disability or death...hypertension, arthritis, depression, etc.
